I would not say that homicidal thoughts, images or impulses are common in OCD but a significant proportion of these patients experience these kind of thoughts.

From my clinical experience there is almost one in 10 people with OCD suffer with these ideas.

The positive thing about them is that like other symptoms of OCD they respond to treatment. Most of the patients show improvement by end of 4 weeks and almost all by 12 weeks. So it can be said that one to three month is response period for harm OCD which is considerably longer in compare to depression and other psychiatric disorders.

Another thing I like to comment on is dosage of prozac. If you would have been my patient I wold prefer to increase the dose of prozac to 60 mg by end of 2 weeks. The reason behind doing so is that the required dose of Fludac in a young person is 60 mg and above.

Prozac along with latuda liklely to help with harm ocd symptoms by end of two months although for complete symtom resolution may take 12 weeks as stated.

In case it does not show complete responseIit can be augmented eith Clomipramine a kind of TCA during later part of treatment. You are requsted to doscuss this too do your doctor can take a call.

The response varies from person to person but most of the patients with OCD require high dosage. In my experience for an averagely built young person dose of prozac appears around 60 mg and above.

I do not have the personal experience with latuda (not available in my country) but the dose of antipsychotic falls somewhat lower range (at least applicable for almost all atypicals). I guess 60 mg will be lower dose range as the minimum effective dose of latuda is 40 mg per day.

Having said this I guess your doctor is actively monitoring you symptoms and will take right decision on the right moment. You need to be aware of the things which may remain untouched during routine consultation.

Unwanted thoughts, images and urges are part of harm ocd and generally co occur with slight dominance of one or all. So it can be said that they do co occur in various mixtures.

Since they are part of same disorder they respond to all treatment directed against the disorder. Prozac helps in all the domains while antipsychotics help the prozac by supplementary mechanisms.

Psychotic symptoms generally take upto six weeks to respond and this is the indication that what you are experiencing are not psychotic symptoms.
