Detailed Answer:
Hello XXXXXXX
I understand your concern for your mothers condition.
I believe the diagnosis is almost right and you saw the correct Specialist, that is the Vascular Surgeon and Dermatologist.
If you were not happy with the Vascular Surgeon then you can see another one.
What your mother has is essentially Chronic Venous Insufficiency(CVI).
CVI can be due to Varicose veins or vise versa.
It leads to ankle hyperpigmentation, swelling, itching, rash and later ulcers that take time to heal. It usually happens after Deep vein thrombosis(DVT). She might have had suffered DVT in the past when she would have had sudden onset limb swelling.
At our Vascular Center and worldwide, this condition of CVI is treated with limb elevation and stockings and ablation of incompetent veins.
But, to begin treatment I need a Color Doppler of arteries and veins of the lower limbs to know the present status of the limbs. Then only can treatment be started. The basic treatment is limb elevation which should be done in a way that the legs should be above the level of the heart and should be done for as long as possible during the day and night. Whenever she is standing or walking she should be wearing Class II Compression stockings till knees or thighs. We also advice giving Tab Daflon 500mg twice a day.
For the pain, she can take Cap Tramadol two to three times a day.
I can advice more once I see the Doppler report as to whether deep veins are patent or not.
Do let me know and feel free to ask if there are any other queries.
Take care,
Dr Rishi, New XXXXXXX XXXXXXX

You are most welcome.
Detailed Answer:
Hello XXXXXXX
It will be my sincere attempt to relieve your mother of her problem to the best of my ability.
As for the medicine, Ultratab is good for her. You can give her a maximum of three times a day. Try and avoid Combiflam as it would tend to have bad effect on the kidney. You can also try Tab Naproxen which can also help decrease the pain.
I will wait for the Doppler report so I can advice you on the complete treatment.
Take care and let me know the report.
Please do rate the answer whenever you close this question.
Thank you,

Good to hear from u. She does not require Surgery presently.
Detailed Answer:
Hello XXXXXXX
I saw all the reports.
Its strange if she has severe pain. In that case the only possibility is Cellulitis or infection of the subcutaneous tissues of the ankles.
Her varicose veins are of the very early stage and not to worry about at all. She does not require Surgery for that presently. She does not have incompetence of any of the major valves. She has some perforator incompetence which can be dealt with by Foam Sclerothrapy as an OPD procedure. This might give her some relief. But her mainstay of treatment would be limb elevation and Compression stockings for life.
I suggest you meet another Vascular Surgeon who can do the needful.
He would also be able to rule out ant Cellulitis in that area. However, if there is Cellulitis then she would require a course of antibiotics and anti inflammatory agents.
Do let me know if there are other concerns.
And do not worry, she is not suffering from anything serious.
Take care,
Dr Rishi, New XXXXXXX XXXXXXX
