Suggest Medication To Bring Down Blood Sugar Levels And Reduce Weight?

Question: Hi Doctor,
My mother is a diabetic from last 15 years. Even after taking insulin (H.mixtard 20 units in morning and 10 units in night) from last 18 months, her blood sugar levels are too high. It also caused her weight to increase from 68 kgs to 84 kgs.
Random blood sugar is 311.
Fasting blood sugar is between 200 and 220.
A nephrologist that I consulted for her kidney disease told me that there are drugs available that can control her sugar levels and also don't cause weight gain. He asked us to consult an endocrinologist and said that the endocrinologist can stop her insulin medication and prescribe these drugs which will help her.
Can you suggest the drugs that she can take?
I would also like to state that her calorie intake in the last 18 months (period of weight gain) has been less compared to the diet in the 10 years before that.
She is following a low salt diet.
Her usual diet is
1 chapati in the morning.
Oats and buttermilk in the afternoon
2 dosas at night.
Other health conditions:Hypertension, thyroid, heart disease,kidney disease.

I have gone carefully through your concern!
To decide the therapy of Diabetes, some more information are needed;
- The age of the patient
- The mean glucose level of the last three months ( HbA1c)
- The liver and renal function tests (AST, ALT, Urea, Creatinemia).
If she has altered renal or liver function tests, most of the oral antidiabetic tablets are contraindicated.
A very high Hba1c ( over 9%) is also an indication for insulin treatment , but as she is gaining weight oral antidiabetics can be combined if the above mention tests are normal.
You mentioned also that she has thyroid pathologies, would you please describe the therapy and the last thyroid hormone results( TSH, Ft4 and Ft3)?.
