Brief Answer:
Follow the steps as explained please.

Detailed Answer:
Thanks for your images and are self-explanatory.
The salient points and the solutions:
- You have dermatitis all around in the leg and the foot.
*Occasional and initial cleaning with Hydrogen Peroxide will help to remove all the dead skin and debris in the smaller wounds and it is the sub-clinical infection that does not allow the healing.
Be gentle or avoid cleaning with Hydrogen peroxide over the large wound.
Then use Normal saline (which we use for intravenous infusion) to clean off the wounds.
*You have to apply liquid povidone iodine 5% to the whole limb from knee to toes at least twice daily. This will help to sterilize the whole leg and kill all the bacteria over the skin and in the wound.

- The best part is that the main wound on closer inspection has a healing edge that should not be rubbed at all. You may pour hydrogen peroxide and liquid povidone iodine mixture over it but do not touch it say for 7 days.

- Addition of the Multivitamins and Vitamins A and D in therapeutic dosage helps in early and better healing.

This should start showing the results in 3 to 7 days and it is possible that within 1 - 3 weeks the wounds all over the leg including the larger one may have healed.

If there is no healing as expected, you may have to take an opinion of a Dermatologist for the skin condition and/or General Surgeon for a small biopsy or FNAC of the wound periphery to know the underlying reasons and to check for the arterial blood supply.
I hope you are not using tobacco in any form.

Also important is an X-ray of the foot to see whether there is osteomyelitis of the underlying bones.
Test of blood and urine; to rule out diabetes, anemia, hypoproteinemia and to get this corrected if found.

Please follow all the steps and you will be benefited as all my patient do get.
