She is taking 2 Diamicron Tablets and one Janumet tablet every morning before breakfast. Doctor told that these medicines have stopped working. He has prescribed Germer PL2 morning one and evening one. Please suggest what are the other medicines.

I think the best way to control the symptoms would be to start her on insulin. It's better because the sugar control is better and also the sugar control is more flexible once you get the hang of it.
This I suggest more in view of the high sugars that you have stated despite being on medications.
In case you are interested to switch to insulin, please let me know. I can suggest therapy accordingly. If you want to continue tablets, that can be done. But as I said, the results will be less under control.

If you want to control those kinds of sugars on tablets,
You will have to be tough on yourself as well.
I would suggest a calory intake of not more than 1600 K
That may be bordering on 2000C once in a while.
I would suggest you meet a dietician who can explain this
to you in regards to the local dishes and your chosen foods.
Second component would be exercise.
Walking at least 1/2 to 1 hour twice a day will be good.
As the stamina builds up, you can keep increasing the exercise
duration and the strain.
I would suggest to increase to 2 Janumet 100/1000 tablets-
1 in morning, 1 in evening, before food
And try Reclide XR 60 once at lunch. Before changes in dosage medications please consult your doctor.
