Detailed Answer:
Hi,
From what I understand you are currently preparing for total pancreatectomy with auto-islet transfusion surgery. You are looking for options to manage pain post surgery.
As such total pancreatectomy with auto-islet transfusion surgeries are specifically conducted surgeries under profound expertise. These are painful before surgical correction too and most of the patients have relief of pain post surgery, with controlling sugar levels with improved quality of life. Post operatively, we have the following options to treat pain:
1. Epidural infusion of opiods with morpine, or fentanyl on titrated doses post operatively for a duration of 48 hours.
2. Pain control with use of PCA pumps & adequate lock out intervals with morphine with intravenous route
3. Local infiltration or intrapleural analgesia,& lower thoracic nerve blocks with usage of lidocaine, ropivaicaine, bupivaicaine at intermittent or infusions by securing the catheter.
4. Fentanyl patches with slow drug release,
5. Rare uncontrollable pains are controlled by coeliac ganglin or plexus block
I suggest you to discuss these options with your local primary care provider. If the doctor agrees with the options suggested, he/she can write for a referral.
Wishing you for good post operative outcome & good recovery.
any queries please feel free to discuss.
all the best,
take care
