According to his history I don't think so he has any abnormality since he has not had any developement problems and his other activities are good.
Memory improves along with child growth. You need not worry.
Do not force him to write immediatley after reading it gives stress to him.
If he will not write give break for him and try after some time.
Instead of reading or writing you can teach him by showing things or playing word games or memory games. It will improve child memory.
Explain meaning of words and have them to re spell it.
Try to find that which books he loves to read and write. You can give him comics or short stories books with images to read.
First you teach him that how to write his name, family members names and things names those he loves. You need to be patient with childrens.
I also suggest you give him well balanced diet which contains fresh fruits, vegetables and poultry food. Give him plenty of water and fluids.
Food habits and family habits like wativhing TV loudly can also affect memory or concentration.
I suggest you try aforementioned suggestions for one month If you will not see any improvement then I suggest you visit a peadiatrician.
