It is important to know the location and type of valve problem you are having, for which your doctors have suggested surgery. This will help us to know if your heart function is compromised.
Penedure injection is given to decrease the chances of infection in the heart valves and leading to prevention of a condition called endocarditis. Taking penedure will not reverse any valve problem that you are having.
The main valves to get affected in rheumatic heart disease are mitral valve and aortic valves, these are valves on the left side of the heart. The valves can get narrowed (stenosis) or leak (regurgitation). Surgery is done when there is a severe stenosis or regurgitation involving the valves.
The surgical procedure is based on the type of problem you are having and this is known from the echocardiography report.
If doctors have suggested you operation then you need to give a detailed and discuss the reasons. Taking penedure will not cure the valve problem that is already existing. This is because the pumping of your heart is affected and to prevent further complications, a surgery is the best approach.

LA is normal size.No LV segmental wall motion abnormality. Overall good LV systolic function. LVEF 60% . No LV thrombus. Normal RV-RA size with normal RV systolic function. Normal IVC size, collapsing with inspiration more than 50%, thereby suggesting RA pressure of 5 mmHg. ( Mayo' s clinic proceedings.) No ASD,No VSD, No PDA. No LV thrombus or LA/ LV clot.
pericardium:- No pericardium effusion.
Doppler Studies:- Reversal E.A ratio of mitral Doppler spectral flow, suggestive ofLV diastolic relaxation abnormality. Trivial TR with no significant gradient . Mild AR mean gradient across LVOT=41 mm of Hg, V Max= 3.9 m/sec.

Rheumatic heart disease is a condition with clinical symptoms which might be variable and range from mild to severe in nature. It usually affects the mitral valve (atrioventricular valve) or aortic valve on left side of the heart. The valve defects can be of two types called stenosis (narrowing) or regurgitation (leakage).
In your case, the aortic valve is affected with narrowing of the lumen just before the valve flaps (supra valvular stenosis) with post stenotic dilatation. This in simple language means that there might be deposition of calcium in the area superior to the aortic valve with expansion of the segment on aorta below the narrowing. There is also left ventricular hypertrophy (LVH) . Once aortic stenosis has started, the blood flowing through the valve is not sufficient and the ballooning of the segment beyond the narrowing can lead to risks like aneurysmal dilatation and reduced coronary flow. This can be set right only by surgical treatment.
It is best to do the surgery at a young age to prevent associated complications and for better post surgical compliance. You should discuss the proposed surgery with your doctors in detail and try and understande the surgical procedure they want to perform on you. This is because there are many surgical techniques and some of themare having good outcomes.
