Brief Answer:
Seems Yeast infection

Detailed Answer:
Hello. Thank you.

I am Dr. kakkar(Dermatologist). I have noted your concern.

I need a picture of the rash to narrow down the diagnosis.
You seem to have a yeast infection of glans and prepuce. Yeast infection presents as redness, bumps on glans, swelling, itching, whitish deposition and sometimes cracks along the tip of prepuce. Your dermatologist will be able to confirm the diagnosis after examination.

Your raised blood glucose could be underlying cause of recurrence. You need to have a check on your glucose levels else yeast infection may recur and respond slowly.

I suggest you to use clotrimazole 1% cream, twice daily on the affected area. In addition I suggest you to take an oral antifungal e.g tab. fluconazole 150mg once a day for 3 days after consultation with dermatologist.
Tab fluconazole is an anti-fungal and is a prescription medications. Therefore I suggest that you visit a doctor in your vicinity.

Regards
