Question: My granddaughter went to the ER with severe pain on her side,a week ago Monday, they gave her an ultrasound and X-ray and determined that her colon was backed up. They told her to take oral laxatives., it Didnt work,. Since then she still has side pain. Only after giving her a suppository is she release a very small amount thin like linguini. I'm concerned because she hasn't done anything since last Saturday.that same day she went for an office visit with her ped pa. Her pedestrian put her on Miralax. And still no bowel movement. I'm truly concerned.

Brief Answer:
Retention enema will be helpful in her case..

Detailed Answer:
Hello XXXX, welcome to HCM,
I've gone through your query and understand your concern for your granddaughter,

Now, as per your description, since there's no colonic obstruction found, so initial administration of suppositories and oral laxatives was the right choice only..

Since there's no effectiveness yet, you should just go for P.C. Enema/ Salt Water retention Enema without any further delay, it should relieve the fecal impaction..

In an unlikely case, if despite enema there's not enough effective result, then a manual Evacuation by an experienced medical personnel i.e. Proctologist/ G.I. Surgeon should be attempted..

Make sure she drinks a lot of water to stay well hydrated as it'll help reducing constipation in future also..

If pain doesn't subside despite all these, I'd advice to get a CAT-Scan of the abdomen along with few blood investigations like CBC, Serum Potassium, Magnesium and Calcium along with LFT, Amylase, Lipase done to rule out any significant pathologies (which generally doesn't get picked up in Ultrasound)

Let me know how she stays in the due course..

Take Care
Kind Regards

Brief Answer:
follow as already advised ..

Detailed Answer:
Hello XXXX, Welcome back,
Thanks for following up,

As you've already described, "they gave her an ultrasound and X-ray and determined that her colon was backed up" - means there's an element of fecal impaction, which correlates well with the symptoms as well as the ongoing management, so don't worry and go ahead with the further course of management as I've already explained in details in the previous answer..

If pain doesn't subside despite doing all those, I'd advice to get a CAT-Scan of the abdomen along with few blood investigations like CBC, Serum Potassium, Magnesium and Calcium along with LFT, Amylase, Lipase done to rule out any significant pathologies (which generally doesn't get picked up in Ultrasound)
