Question: I am 79 and recently have been experiencing edema in both ankles and feet. I talked to my Dr and he recommended special hose

What else should I do?

I carefully passed through your question and would explain that your doctor has recommended a special hose, because he thinks suffer from chronic venous insufficiency in the lower limbs, which may be the cause of your leg swelling.

I would recommend a Doppler ultrasound of the cervical vessels to rule in/out possible chronic venous insufficiency.

This exam would also help find the best treatment options (wearing a hose or surgery).

Besides, if extreme dilation of the veins are found in the Doppler with possible thrombus formation, you may need to start anticoagulation therapy to prevent possible deep vein thrombosis or even its complications (pulmonary thromboembolism).

I would also recommend avoid prolonged standing up position, which can aggravate this situation.

But, if the Doppler ultrasound results normal (and chronic venous insufficiency is excluded), further tests are needed to determine the possible cause of leg swelling:

- a cardiac ultrasound to examine your heart function and structure and exclude possible heart failure
- kidney and liver function tests to exclude possible liver or kidney disease
- an urine analysis for possible proteinuria.
