a low-grade fever is unusual with either cystitis or pyelonephritis. Cystitis usually causes no fever and pyelonephritis usually causes high fever with rigor. Despite that, if your doctor has diagnosed a UTI then appropriate treatment is indicated. If I were your doctor, I would have sought signs of other causes of fever, just to be sure.
Before starting antibiotics I would recommend an immediate urine culture to find out the causative agent. The urine culture should not delay treatment initiating for long (that is you should do it immediately and start treatment as soon as possible but right after the culture).
The choice of antibiotic depends on the patient's history. Patients without recent exposure to antibiotics or hospitalizations may receive simple antibiotics like amoxicillin-clavulanate, nitrofurantoin or trimethoprim-sulfamethoxazole. Local resistance rates should be taken into account. Your doctor should be familiar with them. For patients with recent exposure to the aforementioned antibiotics or urinary tract complications more advanced antibiotics are required like ciprofloxacin or even intravenous antibiotics. Your doctor who is familiar with your medical history is the best person to suggest treatment options. Cranberry juice cannot treat infections, it may only prevent them.
