Brief Answer:
Could be long term consequence of medication or pathology.

Detailed Answer:

Hi,

I'm of the following opinion:

1. Since you are using isotretinoin medicine for last 1 year and now noticing freckles on T zone it could be PIH (post-inflammatory hyperpigmentation) which people realize once their acne actually clears.

PS. ISOTRETINOIN does make your skin sun sensitive and going outside during peak hours can result in extensive scarring and freckles.

2. Do get a physical examination with a Dermatologist for appropriate care and management.

3. Since you do suffer from low thyroid (hypothyroidism) thus diffuse hyperpigmentation, melasma is a common manifestation of the pathology.

PS. do let me know about your current Thyroid Function test (TFT) report and do upload a snapshot of the area (T zone)

4. I do treat my patient(s) on the line of "Bhrajak Pitta Dushti" along with medicine to improve the metabolism (Agni) and Pelvic Area (Apan Vayu Shuddhi). If interested I can courier you the medicine.

Hope I have answered your query.
