Suggest Remedies For Frequent Urination And Recurrent UTI

default
Posted on Tue, 28 Jun 2016
Question: Following on from a previous question regarding frequent urination and urgency I have a history of these issues, but. the urgency and frequency has gotten worse, since last year, and a different feature i XXXXXXX noticed is that it feels like i can feel when my bladding is filling up. There is no stinging or burning whatsoever when i need to pass urine but i am now in almost constant pain and discomfort in the lower abdomen pelvic areas, and also experience alot of pain and discomfort in the lower back and both sides. I am seeing a urologist and he has ordered a renal and bladder ultrasound which i did, and for a urine sample to be tested. I have done these. He also suggested having a cystoscopy. I started getting urinary tract infections a few years ago and back then the medications that the doctor gave me worked now Each time i would go to my doctor to test a urine sample she would tell me that there were some white cells/ a mild infection in the sample. But every single medication she has given me does not do a thing. It doesn't seem to work. So i have to ask is it really a urinary tract infection or is it possible that it can be something else? The most frustrating thing apart from having to pee more often is the constant discomfort and pain i am in.

Brief Answer:
You have what is called as Over Active Bladder Syndrome(OAB)

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you are facing problem of urgency and frequency of urination since couple of months and all routine tests done very recently like Ultrasound Scanning of abdomen and urine tests are negative for infection .

I shall be happy if you can post me few more details to help me in making comments on your problem
1) Are you Diabetic?

Based on the symptoms and absence of any positive evidence suggesting active infection the cause of urgency and frequency and discomfort in lower abdomen is mostly due to Over Active Bladder Syndrome .

Normally one gets feeling(Sensation) of need to pass urine when the bladder is filled with 250-300 ml of urine .However in OAB there are involuntary contractions of the bladder even after it gets filled up with only 30-50 ml of urine giving rise to sensation of urge to pass urine (Urgency) and one passes small quantity of urine .

Taking anti muscuranic drug like Darifenacin once daily will definitely help to resolve this issue .

Please note that it takes 10-15 days to get relief from urgency and frequency after you start taking medication and has to be continued for 4-6 months .

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.

Hi Dr.
thank you for your reply.
Firstly in answer to your question. I do have a history of Diabetes on my mother's side, but i currently do not have Diabetes myself, but i do have Pre-Diabetes. This has been the case for a while, but the frequency and urgency with urination has been been a long standing issue, but since around last year has gotten much worse. What has me very curious and mystified is if i have a UTI, why won't any of the antibiotics that i have been given get rid of the infection? My general practitioner keeps telling me you have a mild infection but nothing gets rid of it?
what i forgot to mention is that when i have to go out, i notice that i sometimes seem to go to the toilet more often? almost like anxiety or something? i don't know if that is related?
I have heard of Overactive Bladder Syndrome. Is this similiar to Interstitial Cystitis and how do you tell them apart? being in near constant discomfort concerns me as i don't understand why antibiotics won't work if i have an infection like my doctor keeps telling me i have?
i should note that my doctor did prescribe Ditropan for me but it does not appear to be helping with the issue.
the medication that you mentioned, Darifenacin? What does it do, how does it work?
thanks

Brief Answer:
Antibiotics will not help as there is no infection.

Detailed Answer:
Hello
Thanks for follow up .

Antibiotics will be of help if there is infection .OAB is characterized by symptoms similar to UTI but there is no positive evidence of infection either on routine urine test nor on urine culture .

OAB is different from Interstitial Cystitis .Interstitial Cystitis there is abacterial (Non bacterial )infection (Inflammation)of the bladder mucosa without any organisms being detected on urine culture. It is characterized by severe pain during micturition where as in OAB there is no severe pain while passing urine nor there is any inflammation of bladder mucosa ..

Darifenacin is an anti muscuranic drug that inhibits involuntary contraction of bladder muscles and there by prevent urgency for urination.

Thanks and Regards,

Thank you Dr. XXXXXXX
you have helped. The only thing is, from what i understand, Overactive Bladder Syndrome doesn't cause pain, or the patient will not experience pain. I have been and do experience pain. Not when i have to pass urine, but just in general. As i told you, i get lower abdominal pain, pelvic pain and pain in my lower back and sides. So if it is Overactive Bladder Syndrome why would i experience pain if that is not generally the case for OAB Syndrome?
thanks

Brief Answer:
OAB is associated with discomfort in lower abdomen .

Detailed Answer:
Hello
Thanks for follow up .
In AOB there is discomfort in lower abdomen but there no severe pain while passing urine as it happens in Interstitial Cystitis.

Start taking Darifenacin ,I am sure it will work for ou .
