The irritation of foreskin is mostly due to infection of glans penis (balanitis or balanoposthitis) which is mostly allergic.
Gently retract the foreskin and soak in warm water to clean penis and foreskin.
Avoid using soaps as they may irritate the skin and make the inflammation worse.
Topical antibiotic with steroid ointment like Betamethasone and Neosporin (Betnovate-N) for local application over glans.
Take broad spectrum antibiotics small course like CEFIXIME 200 mg twice daily would also be helpful sometimes but for oral antibiotic, you need to consult your local doctor or your family physician, who will know and prescribe it.
I expect that you will start improving with 4 to 5 ointment applications and in case, if irritation persists even after 1 week of treatment,you should consult a dermatologist and get examined.
You may need some tests to determine the cause of the infection.
