Question: I have severe vaginal itching. I have seen my gyn twice. First time I had a mild yeast infection. Things got worse after treatment. I am 66 years old. Had a complete hysterectomy at age 40. Had breast cancer 2 years ago and am on a daily medication to stop estrogen. I have not changed anything in my daily routine. Soaps, detergents, etc. Last time I was at my gyn, she said she saw no infection during exam & recommended I see a dermatologist. Her employee suggested Magee women's hosp. I have seen it many times on my trips to XXXXXXX Where do I go from here? The itching is horrible.

Hi there,

This sounds like Lichen Planus, which is an eruption with characteristic shiny, flat-topped purple papules and plaques on the skin or mucous membranes. Sometimes, there is nothing noticeable on the skin and a biopsy should be done. Itching is usually severe and debilitating. It is believed to be caused by autoimmune destruction of the keratinocytes in the skin or mucous membranes. It is usually diagnosed with appearance, but a biopsy is generally needed if there is doubt and no noticeable lesions to rule out malignancy.

Occasionally, it will resolve spontaneously. Topical steroids such as Clobetasol are needed if it become severe. You can also try topical antihistamines. You doctor can also discuss the soak and smear method if it is severe enough. Alcohol, cigarettes, and spicy foods can worsen symptoms.

I would continue the steroids and try topical antihistamines. You also should have a biopsy to rule out malignancy.
