Brief Answer:
Gastritis or Peptic ulcer. Endoscopy need Kidney disease induced Osteoporos

Detailed Answer:
Hi there.

He could be suffering from Gastritis or Peptic ulcer. He needs to her an upper GI Endoscopy done to look for the findings.

He is on many medication without any antacid. Please give him Tablet Ranitidine 150mg twice a day, before meals.

Since his Serum Creatinine is rising, this is not a good sign and could indicate impending renal failure. This can be associated with Osteoporosis or weak bones which could be causing the bone and Joint pains.

Kindly get him a DEXA Scan of hip and spine to look for Osteoporosis. Consult a Nephrologist regarding his riding Creatinine values.

I hope I have answered your query.

I will be available to answer your follow up queries.

Brief Answer:
Heart attacks can be painless too. Needs a thorough workup

Detailed Answer:
Actually one can't tell if it's just Angina or heart attacks. It is best he consult his Cardiologist and get tested as advised to rule out sinister pathology. There can even be painless heart attacks so any kind of chest pain is of concern especially if recurrent.
