Question: I am having hairfall with bulbs at the end. These are small sand like grains attached to the end of the hair. Also my scalp itches because of it. With itching the hairs fall of more often. Can you please suggest.

I am Dr Asmeet Kaur Sawhney, a dermatologist.
There are many causes of hair loss and treatment depends on the cause.
The most common causes encountered by me are male pattern baldness, fevers like typhoid and malaria, anemia, thyroid disorders, certain medications, chronic diseases including that of the scalp, accidents, trauma and surgery.
You need to rule out all these before starting treatment.
Are you having loss of hair from frontal area and crown area of the scalp?
Do you have dandruff?
For how long are you having itching in the scalp?
For how long are you using minoxidil as it can also lead to dryness and itching?

I am not suffering any chronic disease as such.
Yes I am losing it from the frontal area.
Yes I have powdery dry scalp which gets into my nail when I scratch my head. Also the hairs which fall off have small bulb/grain at the end.
Itching has mostly started after using minoxidil. I have been using it since 6 months.

Management of male pattern baldness

Detailed Answer:
You are having male pattern baldness.

Minoxidil 5 percent lotion is an effective treatment for male pattern baldness.

It has been seen that if taken along with tablet Finasteride 1 mg once daily, the combination becomes more effective.

Minoxidil is the cause of your itching.
So you can use X gain hair serum on the wet hair after head bath.
This will reduce the itching and dryness associated with minoxidil.

Also take antihistaminic like tablet Levocetirizine 5 mg once daily at bedtime for 10 days.
You can even consider going for Dermaroller treatment or platelet rich plasma therapy.

These are new techniques for preventing hair loss but are undertaken at a dermatologist's clinic.
Multiple sessions are required for them to be effective.
