Question: My wife fell against her tibia, against a rock. She was treated for a deep laceration, but since she could walk, no x-ray was taken. Now, almost 48 hours later it is very painful to put weight on the leg. Could there
be bone damage? Should we go for an x-ray?

Pain due to soft tissue injury can increase to a plateau 2 to 3 days after the trauma has occurred in some instances.
This is the reason that after an accident for example, doctors and nurses would say that the pain should be expected to get worse after a few days.

After it reaches this point, it may stay at this level for a few days then start to resolve.

The increase in pain itself is not unusual at this point but she tries conservative treatment for the next 24 to 48 hours with no improvement then she should consider imaging.

Conservative treatment would involve: (1)rest- absolute limited movement if possible
(2)ice therapy
(3)elevation of the leg
(4)anti inflammatory pain medication such as cataflam, naproxen or ibuprofen
One of these should be used at the allowed dosing intervals regularly for at least 24 to 48 hours unless she is in a lot of distress and needs a review
