Question: My question is a bit complex but if you really read it through , it will absolutely clear so bear with me. 1986 I was a federal under cover narcotics agent. I was stabbed in the abd. With a long barbecue fork. Forked over by woman literally. Had laparotomy, three days later became septic and had to be redone. Lost part of my pancreaticoduodenal anatomy. XXXXXXX Roth 2. Took here to resolve dumping syndrome and Hypoglycemia but recovered. 2004, my anastomosis structureizd.Had total gastrectomy. Still was battling periSASodic hypoglycemia and metallic acidosis. 2007 staf infection MRSA mutated into VRSA. Gentamicin saved my but had aka. Lots of severe phantom pain. Slowly got better Thank God! 2015 was going to pick kids in NZ up. Had a dynamic phantom pain episode. My last VA checkups A1c slightly elevated but required any intervention. I over December through mid February tons of night urinarion, thirst, eye sight detection and super, super sever phantom pain and pain in my remaining foot! Glucose is 800 when checked, hospital admission at VA hospital, insulin of course long acting. Novalog before meals. I made up my own sliding scale as ( Honest to God!), poor VA care. Developed immune responses with XXXXXXX log so it was changed to Insulin N.Doctor at VA prescribes no controlled substances, to anyone( I am a professional not a drug seeker.), Glucloses still in 5_6 hundreds and having pain I can no longer handle, I am a disabled combat para- Rescueman so in my wife's word when I say I have pain, it's real! Prescribed cipro for pain and it's not doing g a thing, so this party is almost over! I cannot live with pain this severe. My doctor was running behind as I was telling him this, " how are you coping with this pain?" I replied, "With prayer and tears." He said, " Hope that's working for you.-" I fired him on the spot. Now I must find a competent professional who makes more grounded treatment plans. I don't visit around DR's social groups. What would you suggest? Please give me a truthful answer, as I don't want my wife to be husband less and me out of the world. I enjoy life but I am REALLY taxed with this. You don't need to worry that I might do something bad to resolve this. I just need some help and direction. Thank you for reading this! I am sorry it was so long.

Dear Dr. Kudchadkar; Last night at 5 till 0300, I was awakened with severe pain. as I now was not going back to bed. I drove the our post office in XXXXXXX Mo. There was a mailed prescription of Cipro 30mg EC capsules with a dosage of 1 per day.. I neglected to tell you my medications I was taking. So here they are; Fluroxetine 20 mg capsules (3) (1) time a day. It was prescribed after my leg amputation. I in fact, had it discontinued as it interacts with cipo. Metopropolol (100) mgs a day. My blood pressure gets dangerously high when I have severe pain. Sodium citrate oral solution prn for metabolic acidosis, my long and short acting Insulins which are Novalog And detrolog for long and short Insulins. (Note) with their recommended dosages are my glucose is still remaining 500-700 daily. That alone is why I decided to discharge him for medical needs not being attended to. My allergies Tylenol, Droperidol, XXXXXXX compounds and adavain. I took a dose of Cipro at the post office, when I got home I had a allergic reaction that required a huge dose of benadryl and use of a epi- pen. Now I know I can never take it again. Yes for everything you recommended. I will stand on my head and drink milk on a full moon to get this some relief! How do I get started! I live in XXXXXXX Mo.and have fired the VA hospital for incompetent treatment. Any help would be a Gift of God to get over this! Any suggestion I will certainly do.

I have read your reply very carefully. I feel that the actions that you have taken were right and justified. So, I will guide you professionally and effectively.

I am hesitant to put you on opiates, like oxycodone and the like, despite this, I feel that your case is unique and it's probably justified.

I would ask you to take an opiate (any) with a low dose of naltrexone side by side, this combination will prevent you from getting addicted to them.

You now have to visit a doctor and put forth your case. You wouldn't need to try hard because you have a moral and ethical justification in asking for opiates.

Denying you opiates would be tantamount to violating your rights to XXXXXXX and freedom - from agonizing pain.

I would ask you to visit a website called "healthguides", and locate a US doctor who is sympathetic to you, just as I am. Someone who understands.

Also blood sugar levels of 500-700 despite Insulin therapy is critically high and needed to be addressed at the earliest, so getting examined by the Endocrinologist would be recommended ..

I am positive about a bright future ahead for you.

Think of good things, perhaps all the positive things that happened to you, in your life before this incident. You'll feel better.

You might feel like asking me questions as regards me suggesting that you try long term opiate painkillers.
