Hello, Doctor, thank you for your answer. Unfortunately, gout has also been ruled out. Uric acid levels are normal. Since the patient has had gout in the past, he knows gout symptoms caused by uric acid crystal formations in the joints. This is something different. It it most definitely built-up fluid. But only in one foot, and does not respond to any treatment tried so far. Icing temporarily reduces redness and swelling, but the pain is pretty constant, and he has difficulty walking because of it. I will offer him Iboprofen, but I read online that the use of NSAIDs may be worse for his symptoms.
What else could it be?
What else could it be?
Brief Answer:
This looks classically gout
Detailed Answer:
Hello,
His symptoms are very typical of gout. Normal uric acid does not mean that you can not have gout. Gout needs fluctuation in uric acid so even if patient with normal uric acid level have sudden increase for short time due to high protein intake, fever, stress or physical activity then this attack can start.
Other possibility is of bursitis. So I would suggest you to give him Brufen. If still pain continues then steroid injection in toe can be given at most tender site. it should be given by trained orthopaedic doctor.
thanks
