Suggest Remedies For Urinary Incontinence In A Diabetic

Question: My sister-in-law has diabetes and goes to the doctor on a regular basis and she has such strong urine smell and she can just be sitting and she will urinate all over herself and not know she is doing it what would you suggest

Thanks for query .

Your sister in law who is 67 years old and known to be diabetic is facing problem of passing urine without her knowledge .

The involuntary passing of urine she has is mostly due to retention of urine with overflow incontinence associated with UTI giving rise to strong smell to her urine .

In females as the age advances there is a generalized atrophy of Genito Urinary organs that causes narrowing of urethral passage what is called as Urethral Stenosis .This leads to incomplete emptying of bladder when she passes urine resulting in to significant amount of urine to remain in bladder what is called as Post Void Residual urine (PVR)This stagnated urine is is the source of urine infection giving rise to UTI .

Following few factors contribute to cause incontinence
1) Ill controlled diabetes causing Diabetes Neuropathy resulting into loss of power of bladder muscles .
2) Age related weakness of pelvic floor muscles
3) UTI

She needs to consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Endoscopic Examination of bladder (Cystoscopy) with Calibration of urethra to be followed with self retaining indwelling catheter for 4-6 weeks
4) Ta take appropriate antibiotic as per culture report for 2 weeks .

This should be followed by doing Clean Self Intermittent Catheterization (CSIC)
3-4 times daily to keep her bladder empty .This will help her to regain her bladder tone and prevent her from getting UTI
