Question : My mom had a tube put in her kidney to bypass a huge stone as she is 94 and cannot have anesthesia to have the stone removed. They had to place the tube near the top of her kidney due to the fact the stone was so big. She has been bleeding from the kidney since. They attempted to insert a coil to stop the bleeding, but that failed. They gave her blood and platelets and the bleeding has slowed, but now she has a blood clot in her left arm due to the IV's. In spite of the fact that the kidney is still bleeding, they plan to release her to a nursing home tomorrow. Her vitals are all fine, she is mentally alert, talking, joking, etc. They cannot give her platelets because of the clot. I feel she should remain in the hospital until the bleeding has completely ceased. What is your opinion?

Thanks for your query,based on the facts that you have posted it appears that your 94 years old mother has a big stone in her kidney and being unfit to be anesthetized for major surgery of PCNL the Urologist has done per Cutaneous Nephrostomy (PCN)to bypass and to drain her kidney.
She had profuse bleeding after few days mostly due to Arterio Venous Fistula.(AV Fistula) which is known complication of this procedure .An attempt was made to close the fistula by putting endocoil which unfortunately failed .
In majority of the cases this fistula gets closed of its own without any intervention .
Since she has been given blood and platelet transfusions to correct her blood pressure and blood loss and her vital parameters being normal there is no harm in taking discharge from Hospital and to observe her at home with the expectation that her bleeding which is very minimal at this stage will stop completely.
