Hello ,

Thanks for writing to us.

I can understand your apprehension.I have read your other queries and almost all causes have been ruled out and there seems nothing serious pathology.
3 urologist consultation can not find any reason and still problem is there that really makes you worried. But I also feel there is nothing serious.

Let me know some more details

- Since when this problem is there?

- What is frequency of sex/masturbation in your case and blood is always there?

-Does your sex partner have any urogenital infection?

-Do you have blood pressure or diabetes?

-Do you have habit of smoking or alcohol? if yes details regarding the same.

-Did you have history of any injury or surgery in pelvis area?

-Can you upload photos of semen with blood? (to check its severity)

- Do you/your partner use any lubricant or gel during sex?

-Have you taken any regular medicines in past or taking presently for any purpose? If yes describe the details of medicine with name, dose, duration and purpose.

I will be happy to help further.

I see blood when i ejaculate and also sometimes when i go to toilet(stool) something comes from penis similar to semen. It comes with pink or some dark color close to dark grey or black.

Something may you need to know. Lately i developed a habit may be has something to do with it.

i have arousal and enjoy a woman without penetration then i stop without ejaculation.

is that a problem.

Brief Answer:
Go for semen analysis

Detailed Answer:
Hello,

Thanks for information.Sorry for delayed response.

I have seen all your reports attached in past questions. They are normal and rule out all serious problem.

But I feel still there are two reports to be done.First is Semen analysis.Second is PT, APTT and clotting time for bleeding disorder if you have any tendency of bleeding.

Whatever is coming out during stool passing is also need to be checked by laboratory testing that what is that? Likely it is semen or may be some infection.

No it is not a problem that getting arousal without ejaculation. But yes this thing may be responsible for passing semen while you go for toilet as semen comes out of seminal vesicles but not ejaculated during arousal. While forcing for stool due to same nerve supply this semen comes out when you go for toilet.

Revert back to me with reports.

Take care.

Brief Answer:
these are blood tests for check up of bleeding problem

Detailed Answer:
Hi,

PT(Prothrombin time) and APTT (Activated Partial Thromboplastin Time) are blood test to check coagulation defects and bleeding problems. As you have undergone almost all required investigations these are the only tests remaining to check for abnormal bleeding from the body. However if no other problem of bleeding except blood in semen, results will mostly be normal. But for assurance I am advising the same.

And semen analysis will help to actually check the amount of blood in semen and may be its origin.

Hope this answers your question.

Brief Answer:
reports suggestive of some infection likely

Detailed Answer:
Hello,

Thanks for updating the reports.

Pt and PTT are normal.

Semen report is not normal.Presence of WBC and RBC in semen suggests some infection is likely there in prostate or may be in testis.
You need course of higher antibiotic like levofloxacin and if does not respond to the same, intravenous antibiotic may be needed. To choose specific antibiotic semen culture-sensitivity report has to be done. Consult your treating doctor with reports for further treatment.

You have culture reports as you mentioned. I can not find the same in past records. If it is currently available please upload the same(urine/semen culture report)
I will be glad to answer if any query is still there.
Take care.

Thanks for the reply.

I have done the semen culture before and was negative. I have done again yesterday and it will be ready in 3 days.

The doctor gave me Zithromax and that is finished before I do the test.
He gave me now Tarivid (Ofloxacin) 200mg
and xatral 10 mg

which I have started yesterday.

Please help answering my questions:

1-is this serious?

2-You said infection but could it be something like blue ball. I have been doing a bad habit recently which is arousal without ejaculation. Could this be the reason?

3-What antibiotec you suggest please.

Thanks and I will post semen analysis in 3 days.

Brief Answer:
not much serious problem as no other symptoms

Detailed Answer:
Hello,

Thanks for follow up.

Ofloxacin is good option. If I were your treating doctor I would go for ofloxacin or ciprofloxacin. And if needed doxycyline or nitrofurantoin is the option. The best way is go according to culture-sensitivity report.

Xatral is indicated when urination difficulty is there with prostate hypertrophy. But you do not have nay symptom of that so it is not useful in my opinion.

Your infection is not so severe as well as no other symptoms which make us to worry regarding the same. It is not serious condition but better to treat infection soon.

No I do not think that arousal without ejaculation can cause such problem. Possibility of infection is only there.

Hope this answer is helpful. I will wait for semen culture-sensitivity report.

Take care.

Hi
Semen culture is uploaded. The result is negative which even scare me more.

How can I have infection which does not appear in the culture.

Please let me know
doctor
Answered by Dr. Chintan Solanki (14 hours later)
Brief Answer:
No need to be scared. culture may come negative after antibitic course

Detailed Answer:
Hello,

You have taken zithromax. It may have cleared infective organism as you have gone for testing after the completion of zithromax course. So culture is negative. Inflammatory process after infection persist which resulted in wbc/pus in semen.

If still your semen shows blood continuously, you do not have any problem in urine/urination, and urologist confirmed that there is no physical/pathological finding in prostate; course of one more higher antibiotic should be done in my opinion after completion of course of ofloxacin. I would recommend you to visit your urologist with these reports once for examination.

Hope I have clear your doubts. I would be glad to answer again for any query.
