Brief Answer:
Get him evaluated by a child psychologist

Detailed Answer:
Hello

Thanks for asking on this platform

I can understand your concern.
If he is unable to speak words even at this age the language development is obviously delayed. As the hearing test is normal, hearing defect as a cause of delayed speech has been ruled out.
Delayed speech, poor eye contact, inability to engage socially or emotionally with the caregivers and preference for solitary play;all these are features of autism.
I would like to get him evaluated by a child psychologist for proper diagnosis and management accordingly.
Dietery modification is the mainstay of treatment for constipation.
Toileting programmes and medication are other part of treatment.
I suggest you to increase the fluid intake, using high residue diets(whole wheat flour, fruits, vegetables).Excessive milk drinking may worsen the constipation.
Regular toilet habits are an important component of treatment.
I would also like to get screening test done for celiac disease as it is a common cause of constipation/irregular bowel in this age.

Please let me know if you have any more query.
Wish your child a good health.
Regards
