Question: My menstrual cycle is odd. For about four months it is on time then theres a month and a half to two month gap before my next one. Every cycle is a bit heavy with lots of clotting. Some clots being larger then a quarter. within the last 6 months I have also developed 4ever cramps and constipation, before,during and after. At times it will stop for up to 18 hours then I would cramp up a

clot would start it again,briefly. Can "spot"like this for up to 5 days after the first time it stupid. I will also bleed during sexfor up to 2 weeks after. Help?

Hello,
Thanks for the query,
It appears that you are suffering from menometrorrhagia which is irregular heavy bleeding during menses and also with the history of post coital bleeding, the chances of cervical erosions are also high in your case.
It is suggested that you get a per speculum examination and a pap smear with co testing for HPV done to evaluate the presence or absence of cervical erosions and Hpv which is the likely cause of post coital bleed.
Secondly, important is to get a day 21 serum progesterone levels done along with remaining normal profile including thyroid, prolactin and day 2 serum estradiol levels. Progesterone deficiency is likely a cause for irregular shedding in your case with heavy bleeding. The simplest way to control bleeding before tests are done and case confirmed is to opt for antifibrinolytics like TAB TRANEXAMIC ACID 500mg thrice a day from the day2 of bleeding which can help control the bleeding.
The further management can be based on the labs and evaluation.
