Suggest Remedy For Depression While On Wellbutrin

Question: I need a medication because I get irritable, impatient, frenetic and suspicious. I need one that will not make me gain weight, or have bad sexual side effects. I was on wellbutrin for 14 years but now it just makes me sad. Do you have any ideas?

I have gone through your query carefully and can understand your concerns. The main issue here is that you talk about being suspicious but at the same time you have been on antidepressant and never on any antipsychotics.

The person who really has suspiciousness or suspects on people without any fixed firm evidence needs an antipsychotic to get well. Anti depressant will not help him compeltely.

So, please tell if your friends or family say that you suspect them unnecessarily?

Or, if you feel that people talk about you
or you hear people talking when no one is around

or, that you can hear what others cannot?

Please understand that it is important to rule out psychosis in order to treat you properly. Antidepressants wont help in such a case and will only deteriorate the condition.

I'm not hearing voices and I'm only suspicious of my husband. He is a recovering alcoholic and lately I feel that he is acting erratically. I have asked him to use a breathalyzer and his readings are 0. One year ago I came home and found him almost catatonic. When I finally woke him he didn't know who I was or where. I called the ambulance. Since that time he has been sober. (I think). I am just so tense and feel that I am just going through the motions of life. I work four days a week as a speech language pathologist and pretty much carry the load of the finances for our home. I have never had a psychotic incident.
Our daughter passed away 8 years ago. I have been dealing with that ok.
I just find myself get very irritated at the students that I work with which I try to hide because it's not their fault. I just wish I could have XXXXXXX again. I have much to be greatful for so why am I feeling like this? Thanks for any assistance you can offer.

Thanks for writing back.

Your reply is very clear and informative. It has almost ruled out psychosis completely. You have more of worry and anxiety and it is almost taking all fun out of your life.

You have been on escitalopram, bupropion antidepressant but they are not helping you anymore. If I get such a cse in my clinic (of partial resonse) then I try to change the molecule. I have had very good results with paroxetine or venalafaxine ( whichever is easily available in your area) in such cases.

I would also start therapy sessions along with medicines. Therapy sessions along with drugs help in bringing early recovery and it is more long lasting. Therapy helps in correcting the wrong cognitions and lesser medication is needed in treatment.

You are trying hard to keep up with life despite many stressors, which is really appreciable. I would like to tell you that such people have better prognosis. Patients who keep themselves busy get well fast.

So, you can talk to your doctor about paroxetine or venalafaxine and start it. Also, look if you can take some sessions with some therapist or with your psychiatrist simultaneously.

If there is noone trained to help you with therapy or if finances are limited, then even talking out regularly with some good friend or confidate who is wise and calm can be of help.
