Clinically it is known as Otitis media with effusion (fluid behind intact ear drum). occurs most commonly, in infants and children of preschool age.

1. Decision of myringotomy can be taken
a. Otitis media with effusion > 3 months
b. Recurrent episodes of acute otitis media (more than 3 episodes in 6 months or more than 4 episodes in 12 months).
c. persistent hearing impairment, with hearing loss of at least 25dB.
Time frame can be decided by associated factors (hearing, speech and age).
2. Risks- infection, ear drum perforation, hearing loss, thickening of ear drum (tympansoclerosis) and anesthetic complications.
3. Infants or children always requires general anesthesia to do myringotomy.
