Question: I have Costocondrosis as well as copd the Costocondrosis feels like I have a vice around my chest and causes extreme panic attacks last night i thought i was having a heart attack when my pulse XXXXXXX to 170 do you have any suggestions on what could help me my pulse is 85-90 at rest going up to 114 just walking across the room I'm desperate for answers, i cannot take codeine as i am having a 12 week bout of diarrhea and extreme weight loss investigated at the moment unable to eat solids. thank you

Your symptoms seem to be related to a cardiac arrhythmia, which may be induced by an electrolyte imbalance, related to the chronic diarrhea that you have been having.

Besides an autonomic nervous system dysfunction can not be excluded as a cause of possible arrhythmia. As you refer, you have also problems with swallowing.

I recommend consulting with your GP for a careful physical examination and some tests :

-resting ECG and cardiac ultrasound to examine your cardiac function
-chest X ray study
-lab blood tests (complete blood count, PCR, sedimentation rate, blood electrolytes, fasting glucose, thyroid hormone levels).

An ambulatory 24-48 hours ECG monitoring may be necessary to examine better your heart rhythm trends during the day and exclude possible cardiac arrhythmia.

Anxiety could also be related to your symptomatology, but it is important to perform the above tests first and exclude other possible serious causes.

Regarding your chronic diarrhea, the difficulty swallowing solid food and your weight loss, I recommend consulting with a gastroenterologist and performing a GI endoscopy and other lab tests to exclude a possible GI disorder, including a tumor, an autonomic dysfunction or malabsorbation.
