Suggest Remedy For Headaches And Fogginess After Suffering A Head Injury

Question: Hi, approximately 72 hours ago I fell and hit my head while very intoxicated. I'm a 30 year old male otherwise healthy male. I was taken to the emergency room where about 10 staples were used to suture the laceration, and also had a catscan that turned up negative. The laceration is about 3 inches above me right ear running down my partline. Some of the nurses thought my pupils were different sizes and I might have a concussion but I was released into my own care after 4 hours or so at the hospital.

The next day, I slept most of the day and assumed my fogginess was from a severe hangover. However, today I still felt slightly foggy and out of it, and had intermittent headaches around my right ear, so I went to a good urgent care clinic and they tested my reactions and facial nerves, which they said were fine. I still feel like the entire right side of my head aches (not just the laceration), and my feeling is slightly of as well as intermittent eyelid twitching, although that happens when I get fatigued. I'm not sure about my memory and conducted a full day of work without incident, but again still am not sure if I'm just imagining it or if I'm still foggy.

I'm very freaked out that I could have a slow bleed hematoma or something worse; it is hard for me to compare how I felt at time of the injury because I was so intoxicated I dont't remember anything immediately before or until 10-15 minutes after. Do you recommend seeing a neurologist immediately, or going with the urgent care suggestion of monitoring myself and just taking it easy? Any advice is much appreciated.

Brief Answer:
Plain CT scan of Head and bone window of skull

Detailed Answer:
Hi XXXX,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I have gone through your detailed history and in this scenario I would consider the severity of head injury to rule out any internal bleed or a skull bone fracture(not memorising the things before or after the event-indicates that severity was not a mild one).
I would suggest you to visit an ER and get a plain CT scan of Head without contrast to see for any internal bleed and a CT scan of skull bone window,to look out for a skull bone fracture.

If you have any further questions,I shall be glad to have you in follow-up.

Thanks for your reply. I'm confused because you said you believe the description rules out internal bleed, but then suggest another CT scan? Why should get another one if you feel the possibility is ruled out?I

Brief Answer:
If CT is done,no need to get a new CT.

Detailed Answer:
Hi XXXX,
Thanks for being in follow-up.

I think my message was misinterpreted. I never said- 'I believe that you don't have an internal bleed'.
However, if a CT has been done already and ruled out a hematoma, there is no need to get a repeat CT. Your pain,in that case,is due to laceration and it will improve with anti-inflammatory medicines in a couple of days.
But if a CT wasn't performed then I will request for one.

Hope that I have answered your query. If you have any further questions,I shall be glad to answer else please close the thread, rate it and write a review as your rating will be of help to me.

Okay, thank you for clarifying, I understand. A CT was taken and came badk negative immediately after the accident. I read some studies online about MRIs potentially revealing damage 1-2 weeks post-accident that immediate CT scans do not. Do you recommend getting an MRI mext week or is that unneccesary?

Brief Answer:
MRI is not necessary.

Detailed Answer:
Hi XXXX,
Thanks for being in follow-up.

As CT has already been done,so there is no need to repeat the CT scan.

MRI is not indicated. Not at all.

Please be reassured.

Okay, thank you. My last question is do you see any possibility of ongoing cognitive fogginess/slowness given I've felt as such the past two days? Thank you for your help and I'll of course rate the thread after this.

Brief Answer:
No.

Detailed Answer:
Hi XXXX,
Thanks for being in follow-up.

You seem to have researched a lot and so are correspondingly anxious. There is no intracranial damage,so there should not be any cognitive impairment due to it.
