Suggest Remedy For High BP, Palpitations And Gas Trouble

Question: Hello Doctor,

I have been suffering from gastric trouble and acidity along with anxiety, high blood pressure and palpitation. The gastric trouble also causes dizziness regularly which is extremely bothering.

Nothing has been found in stomach so the doctor has referred to Psychiatrist for anxiery treatment.

By the way, I am 49 and haven't have my menopause yet. Some small fibroids have been seen in my uterus, which might have to be removed if menopause doesn't happen naturally within 6 months, as the Gynecologist thinks.


I am having Pexep 12.5 for last 2-3 months.

Recently I feel that it is causing an uneasiness in stomach due to gas formation with upward thrust.

May I ask if all these troubles (acidity, gas, anxiety) may be due to fibroid or nearing of menopause.

Dear Mrs. XXXX,
I have gone through your query, reports and medical history thoroughly.
I completely understand your concerns.

Anxiety, dizziness and acidity can all be inter-related. Each one of these can cause the other symptom.

But be careful about your blood pressure as you have had transient ischemic attack in past. Anxiety, palpitations and dizziness can sometime be due to high blood pressure also.

Coming to your question, I do not think fibroid can cause these symptoms.
Regarding the surgery of fibroid, if it is symptomatic (heavy bleeding, pain), then the surgical removal is necessary. Otherwise, it will resolve as you attain menopause.

Perimenopausal (nearing menopause) symptoms are commonly hot flashes, irregular periods, breast tenderness, vaginal dryness, mood swings.
Acidity and anxiety are not the perimenopausal symptoms, per se.

I would you to continue with drug treatment as advised by your doctor.
If pexep is causing uneasiness, you can request your doctor to change the drug to other one like escitalopram.

Thank you very much, Doctor.

Our Doctor is advising to increase the strength from 12.5 to 25, or have 12.5 twice a day, which I find difficult. However, if I have Pexep 12.5, I find it tolerable.

Do you think we can do that ? We don't mind if the cure gets delayed that way.

If you find Pexep 12.5mg tolerable, you can absolutely stay on this dose.

I think the reason for dose increment by your doctor would be that,
You are taking pexep 12.5mg from last 2-3 months and still have anxiety symptoms. Increasing the dose will increase the blood levels of drug and thus you might have more relief in symptoms.

However, if tolerability with the high dose of pexep is the issues, you can request your doctor to observe for some more time on 12.5mg.

Can I take 1/2 of Pexep once a day? If not, twice a day, to make it 12.5?

Let me elaborate now:


Yes, I have asked my Doctor. In fact I am communicating with her over email because, she is out of town for few months.

Actually initially Pexep 12.5, though used to make me feel uncomfortable, but that time my blood pressure was high. After 2-3 months, my BP most of the time stays below normal. I am not sure if Pexep helps reduce BP.

Even 12.5 is not much tolerable -- I am having half of it twice a day. Even the 1/2 tab gives discomfort.

My question this time is can I have 1/2 of 12.5 daily? I will somehow manage it.



By the way, earlier, the my Doctor suggested to take PAN D before breakfast in order to tackle Pexep's side effect. However, as I am already having gastric trouble, I have been taking OMEZ FF 40 (Gastroenterologist's prescription) before breakfast and also before bed. So, PAN-D might be redundant. I have asked her that already.




So, to sum up, I can handle 1/2 of Pexep 12.5 (difficult to halve it, though). However, if you don't think so, is it okay to take 1/2 of that twice a day?

pexep 10mg is also available.

Dear Mr. XXXXXXX

First thing is, pexep 12.5mg or 25mg tablets should not be cut into half because these are continued-release formulations. Cutting these tablets may cause erractic absorption and effects or even nullify the effects.

Pexep is available in 10ng and 20mg immediate-release formulations.
So, instead of cutting 12.5 mg or 25mg into half, you can use 10mg or 20mg preparation and can cut those tablets if needed.

I guess doctor should have mentioned this.

Yes, pexep can cause gastric upset. You are already taking Omez for it. No need of PAN-D then.

If pexep 12.5mg has been relatively tolerable to you but you still want to reduce the dose, I would suggest you to first try pexep 10mg. If this suits you, then you should continue with this dose.
If not, you can half this tablet.
