Question: Hi Doctor,

My baby boy is 32 days old and a C-section baby. His weight was 2.45 kg and current weight is 3.1 kg. We have just changed formula feeding from Pre-Nan to Nan along with breast feeding. His current medications are "Visyneral Zinc" and "D3 must" along with Sebamed baby lotion.

Now for the past almost 2 weeks he is having irregularities in passing tools; he has started to do it after every 2 days although he did it every single day earlier. Now it's been 2 days he has not pass his stool and did heavy vomiting today morning. I am afraid if he might have started to suffer of indigestion. Please recommend the needful and any medication should I use in this situation.
Thanks
XXXX

Brief Answer:
This might or might not be normal.

Detailed Answer:
Dear XXXX,

I am heartened by users like you who are accessing HCM regularly to address their medical doubts. First of all, congratulations to both you and your wife on the arrival of your son. May he live long!

My short answer mentions that your baby's problem may or may not be normal, but let me explain further. You see, almost 10-20% of normal infants suffer from irregularity of stools in either direction; while some get loose stools from time to time, others land up with constipation for no discernible reason. Before we call this a normal event, we need to satisfy ourselves that nothing ABnormal has taken place. In babies who are only on milk, abnormal dietary events are most unlikely. It is very natural for parents to think that the "milk caused it". However, we must remember that your son WAS using the same milk since some days but NOT having any problems. If we assume that the milk was, indeed, a cause, we can safely check this by changing the milk for a few days. For example, you can shift from NAN to Enfamil A+ and see what happens. If the change in milk helps him to return to normal, you can then stop worrying.

However, before you change the milk, you should WAIT for at least a few more days. During this time, you can use COLICAID drops as these help to expel gas and stool contents. A dose of up to 7-8 drops thrice a day should be adequate.

Let me know what happens after using these drops at least for 2 days.

His mother need not stop or add anything special from or to her diet (assuming that she is breastfeeding too.)

With best wishes for a quick recovery,

Dr. Taher

Hi Dr Taher,

Thanks for your consultation. I am not even in hurry to change the milk. But the medicine you suggested can be used directly or by mixising with water or so. Is there any side effect expected of it?

There is no need to dilute it

You may use Colic aid directly from the bottle as it is completely safe and has no known side-effects.

Brief Answer:
It looks asymmetrical but may be okay

Detailed Answer:
I agree there is some asymmetry between both the feet, but as the baby is just over a month old, he will need to be followed up to see the appearance of the feet after some months. At this stage, you should probably get it seen either by a XXXXXXX experienced doctor or a pediatric orthopedic surgeon in XXXXXXX I would not do any X-rays or other tests at this time unless there are red flags like swelling of any other part of the body, pain on pressing the foot, formation of a small pit at the site where I pressed the foot, or redness or fever.

Brief Answer:
This seems to be related to B.C.G. Vaccine

Detailed Answer:
Your doctor is right. This is the normal response to the B.C.G. Vaccine. Don't worry. It will heal all by itself, leaving behind just one small whitish scar.
