Brief Answer:
Keep the area clean, dry and ventilated. . . No surgery required

Detailed Answer:
Hi young man.
Thanks for your query and the pictures.

I can understand the situation you are going through.

Before I answer your query I would like to tell you about the Physiology (normal functions) of the scrotum which is so much important for the normal production of the sperms by the testis.
- This is one of the two typical / peculiar structures in the human body that has the inbuilt muscle called Dartos just under the skin and cremaster muscles suspending the testes from the abdomen. As you may know very well that the muscles has the function of movement by contraction and relaxation.
-The temperature of the scrotal sac containing the testes has to be 1 to 2 degree celsius less than the normal body temperature for the normal production of the sperms by the testes.

-This is possible by the following factors:

First of all, the corrugated skin of the scrotal sac allowing it to retract or get elongated easily as per the temperature outside.

Secondly, The Dartos and Cremaster muscles makes this possible by contracting if exposed to the cold weather so that the testes are brought near the body to keep the temperature near 35 to 36 degree celsius. And in case of exposure to the higher temperatures the Dartos and the Cremaster muscles relax automatically to have a stretched scrotal sac so the the extra heat is dissipated to keep the temperature of the testes to 35 to 36 degree Celsius.

I hope you understood that the elongation or shortening of the scrotal sac is an automatic phenomenon of the body for the normal functioning of the testes to produce normal sperms.

This is the reason that the changes in scrotal sac are seen in all males.
Since you are are young, 25 years old and not yet married , there is no point in any surgical procedures to stop this normal and most required phenomenon.

The irritation may be caused in your case due to the sweat not being able to get evaporated. Or there may be a Dermatitis casing the irritation.

I would advise you the following:
Clean the scrotal area with plain water 2 to 4 times in a day, particularly in hot and humid atmospheric conditions.
Use scrotal support.
Use loose clothes which can allow the ventilation of the area.
When at home use the undergarments which are very loose and allow the ventilation of the area.
See whether the skin of the scrotum has red or black patches like fungal skin infection. If present, you may please consult a Dermatologist to get this treated.

So no surgery is to be done at the moment.
You just need to keep the area clean, dry and ventilated and your problems may be solved.

I hope this answer helps you, please feel free to ask for further clarification if you need to or if you feel there is a gap-of-communication.

Brief Answer:
No medication available- avoid surgery at the moment.

Detailed Answer:
Let me again assure you, that the scrotum of this position / length not at all be uncomfortable for your partner.
If you keep yourself in cold atmosphere or at least avoid the hot and humid atmosphere there will be regression back to normal.
In my opinion, anyone who is young, getting married should not allow any surgery around scrotum at all unless indicated. Surgery has its own complications.

Alternative way to get your scrotum tight is have scrotal bath in cold water and see if it is getting tight or not. If it gets tight on exposure to cold water, it is sure to go back to normal if you are not exposed to hot temperatures.

It is not safe to have scrotum lift surgery or scrotum reduction. It may not only have impact on sex life but also can cause problems with sperm production, which is the most important part of married life.
Why should you take such a big risk for a small problem.

There is no medication for such a purpose, please do not try something that may cause skin problem as you already have irritation as per the history you have provided.

May I please know your city of residence / state / atmospheric conditions in your city and your nature of work so that I may be in a position to suggests you something to help the problem you are facing .

I hope this clarifies your query. Please feel free to ask for further relevant questions if you need to or if you feel there is a gap-of-communication.
