Suggest Remedy For Lower Abdominal Pain And Enlarged Prostate

Question: Lower abdominal pain for over 6 months. Occurs daily, but not while sleeping or first thing in a.m. Most discomforting later in day/late afternoon/early evening. Under Doctors supervision - tried acid reducer, probiotics, Nexium - no relief, except when first on the acid reducer, and then when on Nexium for first couple of weeks. Tests - abdominal cat scan showed only mildly enlarged prostate with some possible bladder infection (antibiotics worked - then pain came back after 3 weeks); urologist not concerned (no real problem or pain urinating); endoscopy was negative; pancreas and liver ultra sound reveals "fatty" liver (I am a very moderate drinker), Dr suggested diet change, exercise; colonoscopy is scheduled in three weeks previous one was 5 years ago, no concerns revealed.

Brief Answer:
Could be due to Malabsorption Syndrome.

Detailed Answer:
Hi

Greetings from Dr.Divakara.P
Thanks for posting your query.

There are many causes of abdominal pain. But in your case you have been evaluated for most of the causes.

You mentioned that you had urine infection and treated for that. I need to know whether a urine culture test was done before completion of treatment as it is very much necessary.

If a repeat urine test doesn't show infection then you need to be tested for Malabsorption syndromes where there occurs improper digestion and absorption leading to abdominal pain. A simple way to know whether there is malabsorption is to do a 24 hour fasting and see if the pain comes down.

Kindly do it and let me know what happens.

It's been 30 hours since fasting (no food) began, with only intake of water and plain tea - discomfort has not diminished significantly, occurring as before. I had not taken medications during the fast - Nexium and savastatin. Urine culture test was not done, per your inquiry. Also, fyi, bowel movements have been regular, varying somewhat in texture and color, no diarrhea, no blood. No blood or burning in urination. Urination is very slow in middle of the night, up once or twice, but without difficulty during daytime. Discomfort location, to be more specific, is from navel down, but not in pelvic area - perhaps in what might be called the "beer belly" area.

Brief Answer:
Get a biopsy while Colonoscopy and get Stool test also

Detailed Answer:
HI

It seems unlikely that you might be having Malabsorption syndrome but still it is better we evaluate for malabsortion . Since you have been posted for Colonoscopy ask your doctor to do Biopsy too and check for Malabsorption syndromes .

The other possibility is Parasitic Infestation of your intestine . For that you need to get Stool microscopy test looking for Ova and cyst.

One more possibility is Lead Poisoning for this you need to get Lead levels in your hair and blood . But you don't have any other symptoms of Lead poisoning so I am not very much in favour of that. But if all other investigations come normal we will do this testing for lead.

Hope this information was useful to you .

Any doubts feel free to ask .
