Anal Fissure and anal stenosis can be responsible for pain associated with defecation. Both can occur as complication of the hemorrhoid surgery. Local application of either nitroglycerin ointment / GTN or Nifedipine will be helpful in relaxing the tone of the internal sphincter and lessening the associated pain. Try not to strain too hard as the hard stool can injure the anal mucosa leading to further fissuring and may complicate the overall healing. Oral antispasmodic medication like Buscopan plus may also be taken. Some other measures would also prove to be helpful and should be taken note of.
Stool softening laxatives which you are using like Miralax should be combined with local suppositories. Lactulose can also be added to the regimen in addition to miralax. Adequate hydration should be maintained drinking water- at least 2 liters a day. Constipating diet should be avoided with increased fiber intake. Steroid containing ointment can be used to prevent any scarring associated with proctitis.
