Suggest Remedy For Pain On Penis And Retraction Issues

Question: Hello Doctor,
For past one month the skin at the tip of my penis is paining while pulling back and has cracks formed.
even redishness is there at the tip of head. There is no itching only the tip skin has become rough and has cracks. Its getting reduces when washed with warm water and while applying Betnovate N but not getting cured completely. Please advice

No there is no discharge over the same.

There is no itching or burning over the same.

I am applying Betnovate as self medication.

I hadn't gone for blood sugar check in last few months or weeks.

After posting the question on looking into some cases in your site. I found its recommended not to use soap while cleaning the affected area. Last night and today morning I hadn't used soap and I found the tip of head (helmet) is better. The white fungus like appearance was reduced.

Hello,

Welcome again and thanks for posting your details along with pictures. After reviewing the complete condition I can get that you have the condition known as “Balanoposthitis”.

TO tell you simply regarding this condition: it is the infection and inflammation of the penile foreskin and the penis head. There are multiple causes of the infection but mainly it is due to growth of fungus and then superadded bacterial infection.

I can get from the pictures that you have red ulcers and the cuts at and around the skin of penis These infections are common type of infections which more commonly affects the areas which are moist areas.

More specifically this is related to the hygiene especially in uncircumcised males. It is more common to find moist area at glans penis and frenulum (skin covering the glans penis) area which favors the growth of fungus and bacterial proliferation.

Though this infection can occur in healthy individual but raised blood sugar is also very common cause and resultant skin peeling, cracking of skin which hurts during urination and sex.

For the management of the same kindly follow the suggestions:

- Wash penile area for two times in a day with simple water. Be very gentle in washing. Wash it very gently but properly. As you are uncircumcised so it is quite important to retract the skin fully backwards and then wash the area.

- If you are sexually active then after every sexual intercourse too, wash the area properly. I personally suggest you to use protection in form of condoms during every intercourse.

- It is quite very important to check for infection in your partner too. Check if your partner has white discharge then it is the most common cause of recurrent infection. In all the cases of candidal and mixed infection we always treat the partner also. So get your partner treated for the same.

- Wear cotton undergarments. Change these two times a day. Do not wear any synthetic or tight garments.

Betnovate is very strong cream and contain high potency steroids which have the tendency to suppress the inflammation only. It has got no role in the control of infection. Rather if this cream is applied then it can cause rebound increase infection. I personally suggest you to not to use this cream as can harm you rather than benefitting.

- In my patients I suggest them to apply anti-fungal creams such as "Sertaconazole" or "clotrimazole" and take anti-fungal tablets such as fluconazole according to medical advice.

-Check your fasting and post prandial (after meals) blood sugar level. You can take further guidance from a dermatologist and get all the medications prescribed. Do not take self medications.

Overall it is nothing to be worried about. It is a common condition which we see quite often in our day to day practice. With complete treatment of self and partner and with good hygiene maintenance it can be controlled very easily.

In case if you want second issue then there is no harm in not using condom after the treatment of the infection. It would be better if the infection part in you and your wife is completely cured.
