Pain is most likely due to chondrosis.
Detailed Answer:
Hello
I have gone through your findings in detail.
Pain is most likely due to chondrosis(degenerative changes in chondral knee cartilage) with full thickness fissure.Findings are most likely related to history of trauma.
Treatment of full thickness fissure is debatable.
You may need arthroscopic surgical intervention as you have severe symptoms.
You may need bedridement and removal of chondral loose bodies.
Osteochondral autografting (mosaicplasty)can also be considered after evaluation. It demands highly skilled surgeon.
You can discuss it with your orthopedician.
It is good that graft of ACL is intact.

Full chondral fissure is difficult to manage due to limited blood supply.

Hello
Sorry for replying late
Earlier your mentioned that you had injury during duty in sept 2014.Probably findings are related to that past injury.Trochlear chondrosis development in such short duration also indicate traumatic etiology.
Full thickness chondral fissure doesn't respond well to conservative treatment due to limited blood supply to cartilage. So,symptomatic chondral fissure is generally advised for surgical debridement.
