Suggest Remedy For Persistent Fever And Severe Fatigue

Question: I've had a low grade fever fatigue and other symptoms for about two months. The past couple weeks have been very bad. I've had problems with comprehesion, depression and concintration. I went to the er for thoughts of suicide. they tested blood and urine. my urine had a wbc of 80 and 2+ for leukocyte esterase. my blood showed slight hemolyzed (they checked that twice ) bun? creatine gap at 32 does this mean I've had a uti for a long time and it's moved to my kidneys? I've been on antibiotics for a couple days and the mental symptoms seems to be getting better. could the uti have caused all this? I'm 54 and post menopausal for 10 years. . (if that helps ).

Thanks for your query ,based on the facts that you have posted it appears that you have Chronic UTI.This could be secondary to post menopausal Urethral Stenosis which is common in females during postmenopausal period.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have burning urination ,frequency for urination ,feeling of incomplete emptying of bladder etc.

Please consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Endoscopic Examination of bladder(Cystoscopy)

Please take broad spectrum antibiotics like Cefixime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water .To keep your urine dilute This will help to control dysuria.

Further treatment will depend upon result of these tests and final diagnosis. He may advise you for endoscopic calibration of urethra which should be followed by self intermittent catheterization at home for few weeks .

Rest of the symptoms are mostly due to anxiety and can be resolved with the help of Psychiatrist .

thanks for the quick reply, as far as I know I hadn't had any symptoms at all except for the mental issues that came on suddenly a couple weeks ago. I've never had any burning or change in frequency in urination. I did have a mid urethral sling put in a year
ago along with a hysterectomy and prolapse repair. I am currently on cephalexin 500mg twice a day. This is my 4th infection in a year. All where found while looking for something else. I don't understand how I can keep getting these infections and not have any symptoms except for odd behaviors. can a uti really go to the kidneys and blood stream if untreated? see I can't say how long I've had this one and I kinda worry about next time.

Thanks for follow up .
Post Menopausal Urethral stenosis is one of the important factors to cause repeated UTI in females .I would still suggest you to rule out the same by consulting Urologist and doing Cystoscopic evaluation .

In the mean while continue taking antibiotics suggested in my reply earlier .

If left untreated UTI can spread retrograde and involves kidneys causing Pyelonephritis .
