I have noted your concerns including medications tried. I will try my best to help you recover with my experience of dealing with similar complaints in the past in large numbers of patients.
I would suggest you take the following approach to the problem which to me appears to be due to excessive moisture giving rise to a fungal infection.
1 Keep the part dry and well ventilated so that excess of sweat does not accumulate in the area.
2 Broad spectrum and safe antifungal that i have found to work very well is FLUCONAZOLE . I advise my patients to take 150 mg every five days anytime of the day after food
3 Miconazole cream or solution twice a day to the affected part helps in enhancing the antifungal efficiency.
4 oral LEVOCETIRIZINE 5 mg a day or a comparable antiallergic helps control itching in the most.
In addition to these I would suggest getting a stool examination done to rule out worm infestation. I presume that there is no cause inside the anal canal.
With these measures, one is likely to start experiencing relief in a couple of days.
I hope you are not a diabetic. In case the diabetic status is doubtful, please get blood tests (fasting and PP) done to know the exact status.

In view of the fresh information, I think for the first few days, local application of Miconazole and Clobetasone twice daily will lead to quicker recovery in your condition.after three days one may switch over to plain Miconazole
Levocetirizine is an anti allergic. If somehow it is not available then you may consider an alternative.
Miconazole and Fluconazole are antifungal agents.

Clobetasone can be substituted with hydro cortisone which has greater likelihood of OTC availability.
It is less likely that problem can go away without proper medication
I would prefer that vinegar containing baths be avoided as acidic substances cause damage to the already delicate skin of perineal area

Thanks for your information.
I have checked the products and their availability in USA.
I found that Cetirizine, Miconazole, and Hydrocortisone are available either Walmart or at Walgreens.
Fluconazole is available as prescription drug. Its substitute is Terbinafine 250 mg twice daily for 7 days.
