Please find my observations below.
1. An enlarged liver can be due to many reasons. If your sister is healthy or obese then an enlarged liver is acceptable. If she is thin built the probably the liver enlargement is from an infection. The bright liver echotexture is from increased fat content in liver and not always a serious problem.
2. The left sided hydronephrosis show that urine is not flowing smoothly from left kidney to urinary bladder. In such situation, there can be a small stone or any urinary infection. No stone is seen in the scan but there can be hidden stones in the lower ureter region. However no definite conclusion can be drawn if this is the cause of her pain, without a clinical examination.
3. There is an anterior abdominal wall defect seen in the scan through which there is fat herniating. This is because of weakness of the muscles in anterior abdominal wall. The report mentions it as reducible hernia therefore it must be a small hernia. This is to be discussed with a surgeon and to know whether any surgical correction is required.
4. From the above findings, a clinical examination is required and I hope her doctor has done it and come to a probable cause of the pain. From the scan report alone it is difficult to know the cause of severe pain unless you write with more details on the exact area of pain and the duration, aggravating and relieving factors and if it is related to food.
