Your fresh history can be rarely attributed to drugs which you are using for your connective tissue disease.
As the current symptoms are really acute and you are using those since long so such symptoms are less likely due to these drugs or due to acute flare of the pre existing illnesses. Such acute history can be due to some infective etiology leading to gastroenteritis. Unless it doesn't persist for weeks or months it can't be related to menopausal or connective tissue disease symptoms at this moment.
Ideally you can use soft bland diets for a while and plenty of fluids preferably with Oral rehydration salts as extensive sleepiness and lethargy can be due to water or electrolytes loss with diarrhoea or vomitings. Using antispasmodic drugs like Drotaverine or Hyoscyamine may help with cramps.
If symptoms persist, you can discuss using antibiotics like Ciprofloxacin along with Metronidazole with your GP. Extensive blood work or stool examination can wait for a while.
