Dear Ms
Its unlikely to be result of Surgery for removal of Bartholin gland. The nerves controlling the deep rectal tone are situated more deeper and unlikely to be damaged during the Surgery.If ever the nerve were damaged the tone would not have been increased.
You seem to suffering from Chronic constipation , possibly underlying anal fissure needs to be considered( Small ulcer at anal opening which causes anal muscle to go into spasm )
Chronic constipation in an adult is likely to be due to following
1. Low fibre diet
2. Decreased intake of water
3. Presence of lesion of anus and rectum
Before embarking on blind empirical treatment its advisable to undergo thorough evaluation by General Surgeon/Gastroenterlogist
I would suggest you the following
1. Plenty of oral fluids: 2-3 liter of water a day
2. High fibre diet: 2 servings of vegetable salad and fruits everyday
3. Oral Laxatives
4. Daily exercise especially 30 minutes of brisk walk
I would also suggest you to consult Physician for primary assessment, if that would take long time then you can send me clinical photograph for further assessment
