For a young man of 45 years to have the complaints which you have described is unusual but not rare. I am not aware of the medications which you have been prescribed but you could benefit from what we call a trial of alpha-blockers. These drugs open up the outlet, and relieve hesitancy which is what you have. they work well if you have bladder outlet obstruction. Sometimes, the you will need tests like Urodynamics to confirm if you do have any outlet obstruction, but I would rather reserve the same if you do not perceive any benefit at all from use of these drugs.
You can start with drugs like Alfusin / Alfoo 10 mg once daily at bedtime and see if things get better after a week or so. These are usually well tolerated, and except for mild postural giddiness (especially when you shift from a lying to standing position), they have a very acceptable side-effect profile.

now i am also feeling mild pain or a peculiar contraction (tension ) in penile muscles till remain 1-2 hours in a day . is here alpha blockers are useful to solve this problem ?

I did not anticipate that at your age you will have a positive finding on ultrasound. This usually is helpful in excluding other conditions such as a bladder stone where patients could have similar symptoms. It also helps us indirectly by letting us know if there is bladder wall thickening or if there is significant urine which is left behind as residue.
As I said, a trial of alpha-blockers could be useful even if you have intermittent symptoms. If you have no relief or you or not satisfied with the degree of relief, you will be justified in going for relatively more complex tests like urodynamics which will comprehensively inform us if you do have a problem or not. Unfortunately this is a complex test and should be done only in centres with good reporting.
For the present, you can try taking alpha blockers as I mentioned earlier.
