Brief Answer:
Topical 5-flurouracil, Oral biotin and calcium
Detailed Answer:
Hello,
Sorry for late reply.
I have gone through your case reports in detail.
It seems you are having a condition called trachyonychia which means 'rough nails' and when trachyonychia involves all the nails its called "Twenty nail dystrophy".
It very challenging to find cause of twenty nail dystrophy.
In may cases, its secondary to chronic skin conditions like lichen planus, psoriasis, alopecia areata etc or due to some drug reactions or sometimes genetic and idiopathic.
Your nail biopsy rules out any chronic condition like psoriasis or lichen planus nor there is any evidence of fungal infection but there are signs of inflammation suggestive of trachyonychia.
Treatment again is very subjective and depends on any underlying cause found and several regimens have to be tried step by step depending upon the response.
In my experience, I will firstly suggest you oral biotin, calcium and other multivitamin tablets for atleast 3-6 months to take care of any subclinical vitamin deficiency.
I will also advise you application of topical 5-flurouracil and combination of clobetsol and calcipotriol ointment.
If no results after 3 months then I may consider intramatrical injections (Injections into nail bed) of corticosteroids.
In some cases therapy with UV light can also be tried (PUVA therapy).
I would insist you visit your dermatologist again and discuss the treatment plan I highlighted for you.
I hope you get well soon.
Let me know if you need any more assistance from my side.
Thank you
