Brief Answer:
Increase insulin dose & dose of Glycomet SR.

Detailed Answer:
Hi XXXXXXX

Welcome to HCM. I had gone through your question and understand your concerns.

Your diabetes is currently uncontrolled. Only insulin can bring down blood sugars like more than 500 mg/dl. Our plan of action should be

1. To control diabetes

For that you have to increase your insulin doses. If sugars are not getting controlled you should meet your physician. You have to get admitted and changed to four times insulin - 3 doses of rapid acting insulins with meals and 1 dose of basal insulin. Your diabetologist can help you with this.
The Glycomet SR dose you are taking is low. You can go up on it after consulting your doctor if your kidney and liver functions are normal.
Reduce stress - Yoga, meditation, diet, exercise, good sleep all will help

2. To find out why your sugars suddenly went up
It could be due to stress. But other common things should be looked into.
Are you taking insulin injections properly? Look at the injection site if there is any infections/ swelling. How good are you with diet and exercise?
Are you having any other infections like urinary infection or tooth infections which will suddenly shoot up your sugars?

3. To detect any complications of diabetes.
You should consult a diabetologist/ Endocrinologist and do diabetic check up like HbA1c, kidney function, diabetic retinopathy/neuropathy screening etc

The black outs and loosing balance can be there in uncontrolled diabetes which can be improved with control of blood sugars.

Don't Worry. Be confident that everything will be in order again. Most important thing in a diabetic is good sugar control. If it is not proper it will add on to your stress.

Please let me know your lab results, so that I can tell more about your diabetic status.

Hope I have answered your question. If you have any further questions I will be happy to help.
