Detailed Answer:
Hi XXXX,
Thanks for writing in to us.
I have read through your query in detail.
Hoping that you have confirmed your high blood pressure status after getting clinical examination done. High blood pressure is diagnosed if a person has a persistent high blood pressure recording of more than 140/ 90 mm Hg when recorded on 3 consecutive days at the same time and in the same position after taking 30 minutes of physical and mental rest.
Even at home, you must record your blood pressure after 30 minutes of rest.
Amlovas 5 is amlodipine and helps in restoring normal blood pressure. The swelling of limbs is seen in some people who take this medication. If there is persistent swelling even after 12 weeks then it might need to be discussed with your doctors. High blood pressure is usually a chronic medical condition and treatment is to be taken for life. You can also try to take the lowest medication dose and control your blood pressure by taking no extra salt in diet and doing 30 minutes of yoga or exercises every day alternately.
I will suggest you to also get a Renal artery Doppler test to know if you are having high blood pressure due to any problem in reduced blood supply to kidneys. This is required because you are 41 years in age and this condition must be assessed and treated if present.
Please also consult a cardiologist and get lipid profile test done to know if you have high cholesterol in your blood. It is better to get a complete heart check up done because high blood pressure also affects the heart.
Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.
Hope your query is answered.
Do write back if you have any doubts.

Dr.Vivek

Change in medications after visiting cardiologist.
Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.
The BP recording as mentioned by you can be classified as mild hypertension. Amlodipine (Amlovas 5) is a calcium channel blocker and this controls blood pressure by maintaining the calcium ions transport across cells. This medication works well in most patients but can cause swelling of feet and knees as a side effect in many people. Nausea if continuous, is also an undesired side effect.
There are many alternatives to Amlovas 5 considering your knee swelling, if it is severe. This can include giving you a different calcium channel blocker, reducing the dosage, and or adding a known venodilator such as a nitrate, an angiotensin-converting enzyme inhibitor, or an angiotensin-receptor blocker.
Angiotensin-converting enzyme inhibitors have been best studied in this regard. Water pills or diuretics may also be given but they work in a different way by decreasing the plasma volume.
For mild swelling brought on by long periods of swelling, patient can be advised to take rest and elevate limbs but this might not be practical in a young person like you. Discontinuing the amlodipine and switching to an alternative antihypertensive resolve will resolve the swelling.
I suggested Renal artery Doppler to make sure that the high blood pressure is not due to narrowing of blood supply channels in the kidneys. It is a simple ultrasound test and will tell the blood flow to the kidneys.
If you have got lipid profile done in last 6 months then there is no need to repeat it.
We are sorry that giving prescriptions is not allowed online as per ehealth policy guidelines as telling you to take any medication without doing complete assessment might be harmful to you. Therefore it is in the best interest of your health that you consult a cardiologist and then modify treatment after a complete cardiovascular assessment. Most cardiologists ask for a Renal artery Doppler when evaluating high blood pressure.
Please note that if your lipid profile and blood sugar levels are normal then there is no need to stop taking sweets and oily foods completely. You can take it in small quantities. Also please continue your morning walks and this might help in reducing the leg and foot swelling.
Hope your query is answered.

Please continue blood pressure medications. IBS can increase stress and BP.
Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.
It is important to get your blood pressure checked up regularly by your family doctor and adjust medications. Amlovas 5 is a small dose of medication and will protect your system from any further complications of high blood pressure. It is better to continue the medication under supervision of your doctor. If you stop taking medications on your own then the blood pressure can affect the kidneys and eyes. There have also been cases where stopping blood pressure medications had cause bleeding in brain and stroke which is a life threatening condition.
Doing exercise and eating healthy food will also help you but medications are most important because your blood pressure was 160/ 100 mm Hg and that is high. As discussed earlier please avoid taking extra salt in diet and also try and remain stress free.
IBS can increase your stress levels leading to higher blood pressure.

Please consult cardiologist and discuss side effects of Amlong.
Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.
Glad to be hearing from you. Since your blood pressure is slightly above normal I suggest you do not get worried and consult a cardiologist. The will do complete tests including ECG, echocardiography and lipid profile. Also please discuss the issue of swelling in legs after taking Amlong 5.
I also suggest you purchase a good electronic blood pressure machine and make chart of blood pressure taken at home every day after 30 minutes of rest. This will also confirm if you are having white coat hypertension which is sudden rise in blood pressure due to visiting hospital or doctor. This is purely psychological.
Good to know you are taking steps to keep yourself fit. There is no need to stop taking fried food and sweets completely. You can use olive oil for cooking at home and avoid deep fires regularly. Light fries and sweets can always be taken occasionally. Please do not add extra salt in foods.
You can please refresh me with reports of investigations you have till now.
Hope your query is answered.
Do write back if you have any doubts.

Thanks for the email and suggestions.
I had certain questions to be asked.
1. Is there any side effects of Amlog 10 mg?
If yes what are they , i asked the Dr why he has changed the medication and he mentioned that both Amolvas5 and Amlong 10 are of the same parent company and he also mentioned that Amalong 10 is better and with this medication shld come down to : 120/80 easily and this Reading is requied for my age which is 41.6 Yrs and he also told once it come down to 120/80 he will change to Amlong 7.5 MG.
Is this correct??
2. Do you advice me to consult a Cardiologist right now or wait for some time and then consult , please let me know.
3.How long will it take for a High Blood pressure to come down after taking medication. I have started to be under medication from 14 Oct 2014.
Here are the exact steps what i have undergone tilll now.
On 14 Oct 2014 i visited my Family doctor complaining abt Head heaviness and he immediately checked my BP which was 160/100 and he started the Amolvas5 medication immediately.
He didn't do any repeated BP reading once again.
he asked me to visit back him in 2 weeks time , but again went to him
on 2 weeks that is 31 Oct 2014 and at that my BP was : 156/94 .
and he asked to continue the same medication and asked me to meet him again after 2 weeks .
I promptly went before that on Nov 06 , 2014 and that time my Blood pressure was : 145/96 and he continued the same Amolvas5.
I had gone on Nov 06,2014 for the swelling of the keens and i even asked him and he mentioned that it is fine and nothing to worry.
Again today i went and met him he checked my BP which was : 145/92 and he changed the medication of : (Amolgan10.) which i mentioned before and he has asked me to meet again after 10 days.
These are the exact steps i have undergone.
I have a Blood pressure machine at homw which i am not using .
I will start tracking the reading, could you please let me know how many days once i need to track the BP Reading.
I will ask my family folks to use olive oil going fwd for cooking.
Is there any thing i need to do .
Please advice accordingly.
Thanks

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.
Please find answers to your queries below.
1. Side effects of amlong (amlodipine) are increased sleep, dizziness, headache, ankle swelling, oedema, flushing, fatigue, palpitations, abdominal pain, nausea. Rarely, confusion, rash, gum swelling, muscle cramps, breathlessness.
Though amlong will work for you, but because knee swelling other medications can be thought of. There are many medications other than amlong (amlodipine) which can be given to you instead of amlodipine. This needs to be done only after clinical examination and discussion with a specialist.
2. I suggest you consult cardiologist right now for proper guidance and they will suggest you any investigations if required as per your age and symptoms.
3. After taking medication the high blood pressure comes down immediately and might take a few days to get used to the medication. In the beginning small dose is given and after 15 days or a month dose regulation might be required depending on control of blood pressure.
Please record blood pressure 30 minutes after waking up in morning every day when at rest.
Hope your query is answered.
