Brief Answer:
As mentioned

Detailed Answer:
Hi.
Noted your history of thrombosis of Portal vein, Mesenteric vein and Hypersplenism. The pain you explained is very specifically mentioned as constant throbbing pain that lasts for about 10 minutes.

The most probable cause of pain you explained on the right side near kidney and liver area could be:
-Blockage at the level of pelvi-ureteric junction of the right kidney causing classical intermittent throbbing pain that lasts about 10 minutes and goes away.
-Intestinal obstruction of the intestines in this area like hepatic flexture of the large bowel or a part of the small intestine. This can mimic similar pain, within 10 minutes the obstruction getting relived as it may be partial.
-Gall bladder stones causing similar pains
-Inflamed liver or a part of it.
-Pancreatic or bile duct blockage

All the above reasons are thought of as per your classical history.
There can be other rare reasons for which you have to get a clinical examination, investigations and correlation.

I would suggest :
Ultrasonography to find the probable cause, preferably during the attack of pain and for a follow-up of the present problems.
Standing X-ray of the abdomen during attack.
Further investigations of blood, urine, stool and other as may be deemed by your treating Doctor on clinical examination

I hope this answer helps you to go for further diagnosis and treatment.

Wishing you early diagnosis and treatment and great recovery.
