From your history it seems that you are likely suffering from depression. Sertaline has been given to you but you did not improved completely.
Sometime telling someone your feelings is soothing.You are doing the same. You are not able to be involved in CBT or other psychotherapy due to personal circumstances as you mentioned.However in my view every therapist/doctor is ethically committed to maintain confidentiality of the patient and same should be maintained by your therapist also.So try to discuss with your therapist if you feel.
Other option is to write your feelings in a diary and you can feel relax.
I would like to suggest a book 'Feeling Good' by XXXXXXX Burns. It helps in treatment of depression. I have suggested to many patients and many of them improved. It is actually written on basis of CBT. You can easily search on internet and purchase the same.As you mentioned, " I am aware of the "problems" and personality weaknesses carried over from my childhood and have dealt with them with varying degrees of success for 60 years." , I think you would find this book much helpful.
I also would like to mention to visit a psychiatrist if you had not yet. There are many anti depressant medicines available in current days. After detail history and evaluation your medicines can be restarted and you can be improved.
