Suggest Remedy For Weak Erection And Decreased Libido

Question: Hello doctor,

I am 29 year old male, unmarried, haven't done sex till now. I used to have a very good libido and used to get hard erections. But from last 2-3 months my erections are weak, libido has gone down and ejauculates before getting full satisfaction during masterbation. Recently while doing foreplay for the first time with my gf, I noticed I was not getting erection easily and not able to sustain it (we didnt have sex though).
Not sure this is just psychological or any medication required here. Please help me out.

Important aspects of your query are:

• Complaints of: Erectile dysfunction.

In treatment sexual problems first step is to identify cause of the problem. Erectile dysfunction (ED) can be a result of a psychological or physical problem. Although in your case it is difficult to confirm erection problem considering lack of sexual experience. It appear to be performance anxiety induced problem in your case.

Psychological causes: Most common cause in young patients. These include performance anxiety, emotional weakness, inferiority complex, stress, marital or relationship problems, low mood, feelings of guilt, and the effects of a past sexual and emotional trauma.

Physical causes: Include neurological disorders, alcoholism, smoking, hormonal imbalances, diabetes, hypertension, blood vessel disease and chronic diseases.

So, there is need to rule out certain medical conditions which can affect erection. These include diabetes, heart or thyroid conditions, poor circulation, penile venous incompetency of penis and neurological causes. For that comprehensive evaluation and laboratory tests are needed.

Following steps will help you in ED:
-Monitor your BP and keep it in normal range with treatment. So, do not skip medicine doses.
-quit smoking and alcohol (if you are taking any)
-Eat a healthy diet rich in vitamins, minerals and antioxidants: fruits, milk and eggs
-Exercise, particularly aerobic exercise (like: Jogging, skipping and cycling) daily. It will increase blood flow all over body and will reduce stress.
-Maintain your weight within normal limit for your height.
-The commonly prescribed drug by the doctors is sildenafil (Viagra) or Tadalafil (Megalis), which causes erections to be stronger and long lasting, but should be taken after prescription. It is important to rule out medical conditions specially related to cardiovascular system before starting such treatment. Both of these medicines have excellent result in physical causes of erectile dysfunction.

Thanks for the reply, however the information is quite generic for me to resolve this.
FYI:
1) I am fit, exercise regularly.
2) I have the only problem which I mentioned in medical history and no other.
3) I don't smoke or drink

I have consulted a doctor and he has suggested me to take tazzle and ignitor in light dose to improve blood circulation to penis vessel. He hasn't done any tests as such, this was the prescription on just hearing out my problem.

I am not sure though wether I should go ahead with it or not as I don't want to rely on the medicines for sex. Also am not sure wether these medicines will provide permanent or temporary solution. Please guide me in same.

I have stopped taking medicine with beta blocker (refer the medicine mentioned in medical history) after consultation with my doctor though just in case they are causing the problem.

I am really looking for answers to following questions:

1) How do I make out wether its really ED problem or just psychological doubt.
2) If its so then what looks the most probable cause in my case since I faced this problem only from 2-3 months and have also told you my medical history.
3) Is it safe to take the medicine for 20 days as prescribed by my doctor. Will they make me dependent on them in longer run ? Or should I just wait and watch first for couple of weeks since I have already stopped taking beta blocker.

Answer: Psychological issues are diagnosis of exclusion after ruling out possible medical causes. You already have medical risk factor for ed in form of hypertension. Get evaluated for atleast:
Hormonal problems: Serum free testosterone, Estrogen, T3/T4/TSH and prolactin level.
Penile blood supply: Penile doppler study.
Diabetes: Fasting and post prandial blood sugar level.

2) If its so then what looks the most probable cause in my case since I faced this problem only from 2-3 months and have also told you my medical history.
Answer: It is difficult to comment without testings.

3) Is it safe to take the medicine for 20 days as prescribed by my doctor. Will they make me dependent on them in longer run ? Or should I just wait and watch first for couple of weeks since I have already stopped taking beta blocker.
Answer: Before any treatment, in my view, it is better to get evaluated first to know cause of problem.
