I can understand your situation and problem. I have gone through your reports.
Penicillium is fungus. And it is not common fungus to infect.
It is common in immune compromised patients.
So get done Sugar level , HIV etc to rule out diabetes and HIV.
For penicillium fungus following tests are available.
1. IgE level (penicillium specific)
2. Biopsy of sinus tissue and get done histopathological examination.
3. Fungal culture from nasal secretion and biopsy material.
So get sinoscopy done and discuss all these with your ENT doctor.

Thanks, does this penicillium is related to low IgM?
My sugar levels are under controll. Should I try to have Total Serum IgE, IgG,,Precipitin test (IgG)
You please name tests with which Fungal infection of sinus can be make sure, do you find fungal on CT scan pictures, doctors in Canada are not ready to accept that CT scan can show fungal--
Please clarify that does Fungal infection of sinus can travel to blood and bones and also to other parts of body-and can damage brain cells effecting memory?
Does fungal sinus is serious medical concern?

Yes, low IgM levels can cause fungal infection.

Answer to your 2nd question.

No need for total IgE, IgG and precipitin IgG, because your IgE penicillium is high.

Answer to your 3rd question.

For sure and 100% diagnosis of fungal infection, one need to isolate fungus from the tissue and discharge. So ask your doctor to collect sinus tissue and discharges during sinoscopy and send it for histopathological examination and fungal culture.

Answer to your 4rth question.

Fungus an organism. And it is not all all seen on CT scan images. It is seen in tissue samples and discharges under microscope only.

Answer to your 5th question.

Yes, untreated fungal sinus infection can cause fungaemia (fungal infection in blood stream). And this later on can infect any organ of the body including bone, brain, heart etc. And if brain involvement is there, then memory loss can be seen.

Answer to your 6th question.

Untreated fungal sinus infection in immune compromised individuals can be serious medical condition. Are you diabetic?

yes I am diabetic,
to check infection fungal in blood - can we try for blood culture candida /teast you please name it and suggest
present getting /seen that memory is effecting.

today seen walk in doctor- he prescribed KETOROLAC TROMETHAMINE OPHL SOLN- 0.5% for eyes -eyes are painful and swallowed. what do you think this may be due to fungal infection?
On ankles and lower legs skin rashes comes and goes very frequently - using clobetasol propionate cream u s p and some times applying ketoderm, first cream gives more benefit- does these may be due to fungal.
While bowl movement- stools is seen with like fungal stuff ,
skin prick tests comes down positive he prescribed Allergy serum x1 , not have started- suggest does this is required .

Answer to your 1st question.

Do fungal culture of blood to rule out fungaemia. Practically, yield of fungal blood culture is very low. But you can get it done to rule out blood infection.

Answer to your 2nd question.

Your eye problem can be due to fungal infection. But if you improved with ketorolac ophthalmic solution then less likely due to fungal infection. Because ketorolac is simple painkiller and not antifungal.

Answer to your 3 rd question.

Your skin lesions were mostly due to other cause, other then fungal infection because you were improved with clobetosol and this is steroidal cream and not an antifungal.

Answer to your 4 th question.

To rule out fungus in stool , you need to get done stool microscopic examination for fungus. If this is negative then less likely of stool fungus infection.

Those were done for allergy- 21 needles were pricked-
showing allergies for Pigweed,Dock/Sorrel,Russian Thistle,Ragweed,Dandelion,Birch,Kentuchy Grass,Alternaria XXXXXXX grass,Hormodendrum,Penicillium,Cat,Horse,Cattle,Duck Mites D farinae, Duct mites D/pteronyssiuns,Fire Ants,Corn Pollen, Candida Albicans,Wheat Pollen ,Alfalfa.Allergy doctor is making his own Serum under DIN 999981,.but not available any where in Data base, any way - big sea is always dry.
please guide
Actually medical system in this country is most commercialized, no body is there to answer. can you please read Ct scan reports issued by canadian doctors on the bases of CT fimls of CD,s. you will find tons of errors.

I can understand your situation and problem.
Skin allergy is common and should be treated with immunotherapy (like you have beed advised with serum) if skin allergy is recurrent, chronic ,very troublesome and not controlling with routine topical creams.
Immunotherapy is not advisable to all.
So tell me
1. Are you having recurrent akin allergies?
And about CT scan reporting, I believe it differs with radiologist to radiologist. Difference of opinion and inter observer variation is highly prevalent in radiology reporting. So no need to go in such details.
Better to get done sinoscopy for your problem.

1. What are your symptoms? I mean nose related.
2. Since how long you are having such symptoms?

YES you are correct for such difference in opinion.
yes getting skin allergies frequently- on ankles, lower legs,
still if you dont mind please specify those differences of CT scan films, it would help me and our family a lot.
In your opinion -Serum is treatment, but here Allerigist was explaining that this is long term treatment. any way you suggest, does these serums are available in XXXXXXX what is your suggestions for its use--or we can wait for two three months till our visit to XXXXXXX
These symptoms are since long year or long. During last few days- 5-6 days getting severe pain in both of eyes, pain is coming from back of eyes, using Ketorolac Trommethamine ophal solution 0.5 % using it gives more irritaion/burnings , should I continue this or stop it.
What do you suggest - shouls I go to Emergency for eye problems -pain and swelling?

Hello dear, thanks for your follow up question.
I am sorry for late reply as I was busy yesterday.
I have gone through your reports.
You are having obstructive sleep apnoea (OSA), mostly due to nasal problems.
So treatment of nasal problem is must to control the OSA symptoms.
And your CT reports are suggestive of chronic para nasal sinus infection. Recent CT showed improvement then the first CT.
The treatment of allergy with serums is known as immunotherapy. It is recent treatment advance in allergy. And it Ia very well done in XXXXXXX too. So you can get it done in XXXXXXX at the time of your follow up visit. But treatment is of longer duration, atleast 4-6 months. So plan accordingly.
By your history, I think your eye problem is mostly due to sinus problem. So no need to go in emergency for eye problem. Use ketorolac solution.
Get done sinoscopy first and start appropriate treatment.
